Please read and agree to the following terms and conditions of the CAllen Cosmetic Center Botox Membership:

1. 1. Membership Agreement: By enrolling in this membership, you agree to these terms and conditions.
      
      
   2. Membership Fee: A one-time, non-refundable annual enrollment fee of $50.00 plus applicable HST is due upon sign-up and will be processed through PayPal with your first monthly payment. The monthly membership fee of $100.00 plus applicable HST will be charged automatically via PayPal on the same day each month, starting approximately one month after your initial enrollment.
      
      
   3. Included Treatments: The membership includes three (3) Botox or Dysport treatments per year, with each treatment consisting of 60 units. These treatments are to be scheduled at your convenience, subject to availability, with a minimum of 90 days between each of the three treatments. Unused treatments do not roll over to the following year.
      
      
   4. Additional Units: Members receive a 10% discount on any additional Botox or Dysport units required beyond the included 60 units during a scheduled membership treatment. These additional charges will be handled separately at the time of your appointment.
      
      
   5. Product and Service Discounts: Members are entitled to a 10% discount on all retail products and other services offered at CAllen Cosmetic Center. These discounts cannot be combined with other offers or promotions and will be applied at the time of purchase.
      
      
   6. Priority Booking: Members will receive priority when booking appointments, allowing for preferred scheduling. While we strive to accommodate all requests, priority booking does not guarantee immediate availability.
      
      
   7. Cancellation Policy: You may cancel your monthly membership through your PayPal account or by providing a minimum of 30 days’ written notice to CAllen Cosmetic Center. The annual enrollment fee is non-refundable upon cancellation. If you cancel before receiving all three included treatments within a membership year, you may be charged the difference between the discounted member rate and the standard rate for any treatments already received.
      
      
   8. Membership Term: The initial membership term is one year from the date of enrollment. The monthly membership will automatically renew on an annual basis through PayPal unless you cancel your subscription at least 30 days prior to the renewal date, either via PayPal or by notifying CAllen Cosmetic Center in writing.
      
      
   9. Price Changes: CAllen Cosmetic Center reserves the right to adjust the monthly membership fee with 60 days’ prior written notice. Members will be notified via email, and continued membership after the price change constitutes acceptance of the new fee. You will need to update your PayPal subscription accordingly if you choose to continue.
      
      
   10. Medical Suitability: Botox and Dysport treatments are subject to medical assessment and suitability as determined by our practitioners. CAllen Cosmetic Center reserves the right to refuse treatment if deemed medically necessary.
       
       
   11. HST: All fees are subject to applicable Harmonized Sales Tax (HST) as per the regulations in St. Catharines, Ontario. The HST amount will be clearly displayed on the PayPal payment pages.
       
       
   12. Communication: By signing up for this membership, you consent to receive communications from CAllen Cosmetic Center regarding your membership, appointments, and clinic updates.
       
       
   13. Governing Law: This agreement shall be governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein.
